A suggested editable Action Plan template that General Practice can adapt.
An Action Plan should be completed in the event of non-compliance following an audit. The actions should be Specific, Measurable, Achievable, Relevant and Time bound (SMART). The Action Plan should be reviewed regularly to demonstrate continuous improvement. Completed Action Plans should be kept locally for good practice assurance and as evidence for CQC inspections.
